Adds a "Simultaneous Dial" applet. This works exactly like the built-in Dial applet (as of this writing), except that group dialing is performed with all members being dialed simultaneously. The first person to pick up "wins" the call.

- This plugin is not listed at all on OpenVBX.org plugins pages. Can someone at Twilio look at that?

This is heavily derived from the built-in Dial applet that comes with OpenVBX. This was developed by Twilio.
