Executing the release linked-to from README.md as well as cloning the repo and attempting to open the .app directly threw the same message: '"iTermTo.app" is damaged and can't be opened. You should move it to the Trash.'

Looks like you were right, this was a gatekeeper issue. I couldn't use the UI to change my settings because my machine is locked down by my corporate overlords but I was able to get past this by temporarily disabling gatekeeper:

$ sudo spctl --master-disable
$ sudo spctl --master-enable
