File Downloads: Cannot create benefit in edge cases

Received the following video from a merchant. Since we have a lot of file downloadable benefits being created and have not heard about this before, it's certainly an edge case. But creating this issue to try our best to replicate it to avoid others running into the same.

Otherwise, in case we cannot replicate it, it's likely some specific Chrome Extension or similar that's causing issues (seen before) that's hard/impossible for us to debug and resolve.

I am able to reproduce this:

Note that this does not occur when someone uploads first and then sets the name.
