Remove custom Express v5 instrumentation after we bump to OTEL v2

Description

Once open-telemetry/opentelemetry-js-contrib#2801 is released, we can get rid of our own instrumentation and use only this again.

This can only happen once we bump to OTEL v2 (cc @andreiborza )
