Broken `state-surgery` script

The additional storage slot added to the Proxy_EOA here ethereum-optimism/optimism#545 breaks the state-surgery script. Fix it before the next regenesis

We also cannot pull in the OVM_DeployerWhitelist state as the storage slots have changed. We talked about doing this for the previous regenesis and ended up not doing it.
