✨♻️📦 Update all deps and cleanup the consequences

Package

add files to the package.json of each package to make sure we only ship what's needed
move jest, jest-in-case and jsonfile as dev deps of individual packages (instead of having them globally)

AST format

replace loose field in MDAST list item with spread (see syntax-tree/mdast@fa392c2#diff-0730bb7c2e8f9ea2438b52e419dd86c9) and Rework lists remarkjs/remark#364
add meta field to Enki MDAST node of type questionCode (which extends code) (see syntax-tree/mdast@5bf6788#diff-0730bb7c2e8f9ea2438b52e419dd86c9 and Add info field to Code block syntax-tree/mdast#22)
Add a label to reference nodes (see syntax-tree/mdast@dbb1ede#diff-0730bb7c2e8f9ea2438b52e419dd86c9)
