Refactor Application form to reusable Jinja partial

Styles for the post a job form do not match the application form styles for the incoming work on the mvp branch

Task:

Refactor the application form HTML template to a jinja partial to be reused by the post a job form

• changes to post a job form or any other website forms
• style updates

The application form can be found in branch mvp under templates/app/form.html
